Per SC 22 Resolution 03-10, this document cancels and replaces the ballot
contained in SC 22 N3645.  The difference is that this ballot is a combined
NP and PDTR registration ballot only (not also a PDTR approval ballot).

As the text for the NP ballot and PDTR text haven't changed, the ballot due
date will remain 2003-12-11.  For ease of reference, the document is being
re-issued as N3656.
